Our Medical Social Workers are specially trained in providing coordinated care to handle issues that patients and their family may be dealing with. They can assist with:
- Arranging and scheduling in home services
- Provide information about community resources available for the patient
- Finding local support groups
- Assist with matching health care needs to community provided services
- Perform psychosocial assessments, analysis, and counseling
